Community Specialist Panama City 8/25/2020 11/10/2020 12/2/2020

Job Summary:

The Community Specialist is responsible for outreach to identity and  coordinate of resources to nearly homeless and homeless persons.  

Credentials, Skills, Abilities, and Experience:

  • A Bachelor's Degree in human services and 2 years’ experience with special needs population.  Fulfill credentialing requirements per agency policy.

  • Must have good communication and organizational skills and the ability to work effectively without close supervision.

  • Compassion and ability to assist participants to succeed in the program.

  • Administrative self-sufficiency including computer proficiency.

  • Possess valid driver’s license and safe driving record.Must meet insurability standards in order to drive.

Essential Functions:

  • Establish and maintain relationships with prospective referral sources to ensure an on-going pipeline of participants and ensure 100% occupancy

  • Conduct pre-admission assessment screenings to determine participant’s program eligibility. Coordinate admissions.

  • Assist participant with establishing linkages during their stay and discharge.

  • Document interactions with participants and related agencies according to agency guidelines.

Normal Working Hours and Conditions:

  • 40 hour workweek.
  • Occasional weekend or evening as required.
  • Some travel may be required.

Rate of Pay:
Salary $32,600.00.
